- Complete a CAMP online application (see below).
- Sign a camp waiver. Instructions on how to e-sign the waiver will be sent to you via email by our Camp Administrative Assistant and Billing Manager, Lori.
- One of our clinicians will contact you directly to schedule the 45 minute camp intake. Once the camp intake is scheduled, we will charge the nonrefundable camp intake fee ($295) via the payment method you indicated on the CAMP online application, which is stored in our HIPAA-secure online server. A sliding scale is available for the camp intake fee if there is true financial hardship.
- Camp acceptance is based on clinical fit, and will be determined at the time of the intake. If accepted, we will use your indicated payment method to charge the full nonrefundable camp fee, and an email receipt will be sent to you.
- IMPORTANT: You must complete your camp application by the early bird date to be eligible for that rate and must complete the intake process (including the CAMP online application, signed waiver, intake appointment, and payment) within two weeks of the deadline to remain eligible.

Current Clients
- Submit a CAMP online application (see below), including payment authorization in our HIPAA-secure online server, even if we already have a payment method on file for you.
- We will post the camp waiver to your child’s web portal for you to e-sign.
- Our Camp Administrative Assistant and Billing Manager, Lori, will charge the form of payment provided for the weeks of camp indicated in the application and send you an email receipt/confirmation. All camp fees are nonrefundable.
- STEPS 1, 2, & 3 MUST BE COMPLETED BY EARLY BIRD DEADLINE TO BE ELIGIBLE FOR DISCOUNTED RATES.
